Dear AKYC Applicant,

Thank you for your interest in theAngat Kayumanggi Youth Camp 2011, the Ayala Young Leaders Alliance Dumaguete Chapter’s continuing commitment to develop young servant-leaders for the country. We are glad you are considering applying for the AKYC.

Please read all the attachments carefully and familiarize yourself with Angat Kayumanggi Youth Camp 2011 and its nomination and selection process.

We are searching for Filipino high school student leaders in Region VII who are recognized for their exemplary leadership, strong love for country, and passion for positive societal change.

*Each Applicant is required two (2) Mentor / Adviser assessor. Each of these assessments must be sealed in a letter envelope with the name of the applicant indicated in the front of the envelope. The assessor must sign across the flap and submit the sealed and signed envelope directly to the designated School Official handling AKYC.

*Forms 1 to 7 may preferably be submitted as soft copies to the school official in-charge of the AKYC nominations. Soft copies of the forms of the school’s official nominees shall be sent by email to the AKYC secretariat.

*Forms 8.1 to 8.2 MUST BE submitted as HARD COPIES.

Applicants must personally fill up Forms 1 to 7 (computerized forms are allowed) and submit a hard copy or soft copy of these to the school official in-charge of the nominations to the Angat Kayumanggi Youth Camp Secretariat. Forms 8.1 to 8.2 are filled up by mentors/advisers. Please read the instructions for filling up and submission that are indicated in each of these forms very carefully.

Other forms will be prepared and submitted to the AKYC secretariat by the school as part of the nomination process. The submission of documents other than what are required by the secretariat will be grounds for disqualification of the nominee.
